Duties and Responsibilities

Nutrition Consults: Nutrition Consults are addressed within the day following consult request and consult completion is documented in the electronic medical record (EMR) Nutrition Assessments: Nutrition Assessments for San Juan Regional Medical Center (SJRMC) and San Juan Regional Rehabilitation Hospital (SJRRH) patients identified at nutrition risk based on the Nutrition Screen or the Malnutrition Screening Tool are completed within two days and are entered into the EMR Follow-up Evaluations: The plan of care for patients identified as at nutrition risk is re-evaluated and documented within at least 1 week of the initial assessment and as necessary based on the plan of care Nutrition Education: Provide nutrition education to inpatients and outpatients at SJRMC, SJRRH, and the Cancer Center to facilitate self-management of medical nutrition therapy Nutrition Classes: Design and present classes (e.g. cardiac, diabetes, pulmonary, weight management) with current nutrition information for patients, providers, and graduate nursing students. Provide education to patients/caregivers at a level that that is understandable to them. Document education in the EMR within 24 hours of service. Outpatient Medical Nutrition Therapy: Provide education to outpatients at a level that that is understandable to them. Provide documentation of individual medical nutrition therapy to provider office within one week. Community Nutrition: Develop and present timely nutrition topics based on community need. Coordinate with marketing if appropriate and complete the community report form.
