Physical Therapist II - Per Diem

Provides direct physical therapy services to patients and develops individualized treatment plans. Evaluates patients' physical and functional status and implements therapeutic programs.

A minimum of 1-3 years' experience in an acute-Care setting is required, along with a relevant degree in physical therapy. Current state of illinois physical therapist licensure and cpr certification are also necessary.
