School Based Day Treatment Therapist- Charleston County School District

The therapist coordinates the discharge process and performs patient assessments to ensure successful transitions and positive health outcomes. They identify post-Discharge risk factors and provide interventions to support patients and caregivers.

Candidates must hold a master's degree in social work from an accredited program and possess msw licensure by the south carolina board of social work examiners. Certification in case management is preferred.
