Service Navigator

The service navigator provides case management and crisis intervention to support youth and families in preventing child abuse and out-Of-Home placement. Responsibilities include conducting home visits, coordinating referrals, and maintaining professional relationships with service providers and schools.

Candidates must have at least one year of direct experience working with children and families and possess an associate or bachelor's degree. A valid nys driver's license is required, along with the ability to work with diverse cultural and socioeconomic backgrounds.
