Diabetes Educator, Specialty Care Center - Leominster, 40 Hour Days

The diabetes educator is responsible for assessing, planning, and implementing diabetes self-Management education and support plans for patients and their families. They collaborate with physicians and multidisciplinary teams to ensure continuity of care and help patients achieve their diabetes management goals.

Candidates must hold a bachelor's degree in food and nutrition or a related field and maintain registration with the commission on dietetic registration. A minimum of two years of diabetes-Related experience is required, and a certified diabetes educator credential must be obtained within two years of hire.
