Position Summary
Uphold the mission and beliefs of the organization by assuring that all patients receive high quality, comprehensive health education. This position includes, but is not limited to: designing, promoting, implementing and evaluating comprehensive, high quality health education activities that focus on at-risk populations, in order to increase positive health outcomes. This position is also responsible for evaluating the medical and social needs of these patients and connecting them with services as needed.
Qualifications
Master’s Degree preferred At least two (2) years as a registered/licensed Dietician Knowledge, Skills, and Abilities Experience providing education to people among various ethnic and social backgrounds, experience in an outpatient practice setting preferred Excellent organizational skills to manage numerous practice activities Excellent interpersonal skills, team player, professionalism, responsible, courteous and reliable Proficiency in computer skills, familiarity with Electronic Medical Records and willingness to learn all computer systems. Demonstrates initiative, leadership and a high level of educator skills Unconditional ability to maintain HIPAA regulations Demonstrates cultural competency among persons from various ethnic and social backgrounds Leadership skills that include the ability and desire to champion teambuilding within primary Care. Demonstrates fiscal responsibility Demonstrates effective time management skills and follow through Scope of Responsibilities: Work collaboratively with the primary care providers to identify at-risk patients in need of health education as well as broader population needs around health promotion and disease prevention, including diabetes education, weight management, cardiovascular disease, hypertension, renal disease, malnutrition / weight gain, infants with failure to thrive, cancer and nutrition therapy, gastrointestinal disorders, nutrition therapy, sports nutrition or healthy diet, and other chronic diseases. Design and implement comprehensive, high quality group education programs for at-risk patients (and family members) to increase positive health outcomes and reduce high risk behaviors. Ensure content of group sessions are consistently evaluated to ensure high quality and standards of care Ensure measurable outcomes / data are collected and analyzed Provide services to patients of all ages, including: patient education, counseling, assistance in obtaining needed services and supplies. Provide comprehensive , individualized health education and self-management support to patients, in conjunction with the primary care providers Advance the development, implementation and evaluation of the health education programs Participate in health education and promotion activities in collaboration with the Community Health Educator, Director of Clinical Support, the Director of Patient and Community Services, and the Chief Medical Officer. Maintain and submit monthly reports to the Chief Medical Officer that detail encounters, group sessions, and diabetic education activities Act as a liaison/representative at meetings, workshops, conferences as needed Develop appropriate health education materials and programs for use in the health center, in conjunction with the Chief Medical Officer Attendance and participation in meetings, in-services, educational programs and other activities as deemed appropriate Maintain current Health Center policies and guidelines Other duties and responsibilities as assigned
Physical demands
- Travel is required between site (s) by personal automobile or public transportation, reimbursed in accordance with ChesPenn Health Services reimbursement policy. Frequent and prolonged sitting, standing and/or walking may be required. Moderate physical effort with carrying forms, supplies and equipment. Climbing stairs may be required. Stooping and stretching may be required. High levels of stress may need to be endured due to the diverse nature of the care provided.
- Work environment: The primary workplace is a busy multi-cultural ambulatory care setting. There is routine exposure to infections disease and blood borne pathogens.
