Diabetes Educator II

The diabetes educator provides assessment, goal setting, and behavioral intervention to promote diabetes self-Management for patients in inpatient and ambulatory settings. They also document care activities, develop educational resources, and coordinate care with other healthcare providers.

Candidates must hold a bachelor's degree and possess a cdces or bc-Adm certification. At least one year of specialized experience as a licensed dietician or nurse is required.
