Mhs i, (Tip) facilitator

The facilitator provides coaching and support to transitional age youth to help them meet life goals in employment, education, housing, and community functioning. This includes developing individualized transition support plans and collaborating with educators and mental health practitioners.

Requires a bachelor's degree in social sciences, rehabilitation, or education and at least six months of experience in social service or mental health settings. Candidates must be at least 21 years old with a valid florida driver's license and cpr certification.
