Work Shift Job Summary: The Advance Care Planning Facilitator is an active member of the Advance Care Planning program at Wellstar Health System performing the core functions surrounding advance care planning, e.g., consultation, education and evaluation. This individual is responsible for meeting with identified patients to participate in advance care planning. The individual will also be intimately involved in educating providers, patients, families and our community on advance care planning. The position reports directly to the manager for advance care planning and has an essential role in the program. Core Responsibilities and Essential Functions: Advance Care Planning Facilitation
- Conducts facilitated discussions with patients and families regarding decisions for future medical care following the Wellstar Health Systems model of advance care planning
- Ensures adequate documentation of advance care planning sessions in EMR
- Maintains schedule of patient to complete advance care planning Advance Care Planning Education and Outreach
- Prepares and executes unique presentations or publications that reflect the innovations and evidence that will sustain Wellstar Health System as a leader in advance care planning.
- Participates in education of providers, employees, patients, families, and our community throughout the year on the values of advance care planning in variety of venues.
- Advance Care Planning Research Advance Care Planning Infrastructure
- Develops relationship with providers to implement advance care referral process and advance care planning sessions.
- Works with ACP manager to identify, create and review health system policies and guidelines regarding advance care planning.
- Promotes Advance Care Planning as a standard of clinical care across the health system with a focus on Wellstar Health Systems methodology. Works with ACP Manager to improve patient, family, staff and population outcomes based on research, evidence-based practice, and professional collaboration.
- Works with ACP Manager to develop tracking and measurement of outcomes.
- Utilizes process improvement methodology to address gaps in advance care planning process.
- Participates in and seeks out research grants regarding advance care planning. Performs other duties as assigned Complies with all Wellstar Health System policies, standards of work, and code of conduct. Required Minimum Education: Masters Social Work or Masters Other Required Minimum License(s) and Certification(s): All certifications are required upon hire unless otherwise stated. LCSW - Lic Clinical Social Worker GA or LMSW - Lic Master Social Worker GA Additional License(s) and Certification(s): complete respecting choices facilitator and instructor certification required within reasonable time as determined by the Manager of Advance Care Planning. Upon Hire Required Required Minimum Experience: Minimum 1 year experience working in a health care setting or system; Required and prior work with advance care planning Required Required Minimum Skills: Intermediate computer skills including Microsoft Word, Excel and PowerPoint. Must have excellent communication skills and be able to interact with all stakeholders in the health system. Must be able to work with patients and their families in a respectful and collaborative manner. Ability to travel to outpatient facilities within WellStar service area in various locations and work environments. Project management experience strongly preferred. A track record of being results oriented, with demonstrated ability to drive performance is preferred. Ability to measure and interpret results. Demonstrated ability to prioritize work and be highly organized.
