Advance Care Planning Facilitator

The facilitator conducts discussions with patients and families regarding future medical care and provides education to providers and the community. They are also responsible for developing acp infrastructure, policies, and tracking clinical outcomes.

Requires a master's in social work or another related field with a valid lcsw or lmsw license in georgia. Candidates must have at least one year of healthcare experience and prior work specifically with advance care planning.
