Developmental Services Center (DSC) provides comprehensive services across the lifespan-from birth through adulthood-including family development, case management, employment, residential, respite, and community day services for individuals with intellectual and developmental disabilities in Champaign and Ford Counties. Responsibilities

As a Case Coordinator at DSC, you will be responsible for the coordination of a wide range of supports and services for people to whom we provide services. You will need skills in service linkage, advocacy, documentation, securing and maintain entitlements and coordination of Individual Implementation Strategy. Requirements

At least one year of experience working directly with individuals with a developmental disability AND IS ONE OF THE FOLLOWING: RN, COTA, OTA, PTA, SLP, Audiologist, Registered Dietician, Human Services Professional with a bachelor’s degree in a human services field, including, but not limited to sociology, special education, rehabilitation counseling, psychology, recreation, art/dance/music or physical therapies. Knowledge of community resources and a demonstrated capacity to work in collaboration with other human service professionals. Valid driver’s license and reliable transportation HOURS: 40 hour/week, 8:00 AM – 5:00 PM, Monday through Friday; One day/week remote work WHAT WE OFFER YOU: Grant-funded retention bonuses up to 2 payments for a maximum of $1,600 per fiscal year!
