Job Description
Part Time/ 24 hours weekly / 8 hour shifts The speech language pathologist shall evaluate and treat patients; develop a treatment plan; and set goals for patients. Patient and family education is included in the treatment plan and goals. The speech language pathologist I will maintain communication with doctors and other health personnel; and supervise volunteers, students, and staff therapist working on their Certificate of Clinical Competence (CCC). The speech language pathologist I will complete documentation according to applicable national and state regulations as well as hospital and departmental policies. The speech language pathologist I will assist with the education of affiliating students, the department staff, hospital staff, and the community at large. The speech language pathologist I will participate in the departmental operational plan. The speech language pathologist I must intervene appropriately and meet the needs of the patients with impaired mental abilities, vision, hearing, or mobility. Completes appropriate functional evaluation including assessment of pain. Reads / reviews past medical history / inpatient chart. Develops appropriate treatment plans with outcome oriented functional goals including input from patient and/or caregivers. Schedules patients efficiently Provide services to improve functional independence and/or reduce pain Provides ongoing assessment and advances/modifies treatment plan according to patient’s response. Receives and implements therapy orders. Completes evaluation form, treatment notes, progress reports and D/C summaries in a timely manner and appropriately submits to referring provider. Submits written plan of treatment to referring provider per hospital/regulator/payor/practice act guidelines.
Requirements
