The Community Partners Coach is responsible for ensuring the safety and protection of our supported people’s rights as they engage in community activities and seek knowledge.

  • Coach shall do the following: Assist and support individuals in identifying and obtaining employment with community stakeholders. Identifying and training individuals on the knowledge, skills and abilities needed to successfully complete job duties. Guiding and supporting individuals in interfacing and communicating with supervisors and peers. Provide safe and reliable transportation to individuals for work and work-related activities on an as needed basis. Help individuals identify and develop positive relationships from work and work-related activities. Identify and offer suggestions for Individual plan outcomes and job development goals. Identify opportunities for and encourage individuals to contribute to their communities and build positive relationships with stakeholders. Collaborate with vocational services peers to support and guide the community employment goals of supported individuals. Learn and become proficient in the skills of varied professions to support team members in assisting individual in skill development. Be flexible, reliable, and consistent in supporting the vocational service team to assist individuals in obtaining and maintaining employment. Gather and exchange industry-based information to ensure a dynamic pool of skills and abilities to support individuals with new employment opportunities. Positively mentoring and guiding new vocational services team members in learning the skills required for successful job coaching supports. Accurately track and document the attendance patterns of individuals supported in community-based employment. Track and document caseload requirements to include updating and transitioning supported individuals to other vocational services team members. Track and manage employer stakeholder information and communicate regularly to establish employment opportunities. Track and document financial expenditures to include mileage and vehicle usage to ensure fiscal responsibility. Track and report wage differentiations to ensure accurate documentation of Federal and State wage requirements. Ensure effective and efficient use of time and opportunities to maximize employment supports. Complete and submit required documentation accurately and on time. Undertake other duties as assigned, including special projects and cross-program development.

  • Experience: High School diploma is required, Associates degree in human services or related field preferred. One to Three (1-3) years of experience working with individuals with differing abilities preferred. Fluent in Microsoft Office Suite (Word, Excel, PowerPoint, Outlook, etc.) and other database systems.

  • Environment: The physical demands described below are representative of those that must be met by an employee to successfully perform the essential functions of this position. Reasonable accommodation may be made to enable a qualified individual with disabilities to perform the essential functions of this position. While performing the duties associated with this position, an employee (using approved agency lifting techniques) will be called upon to support and lift an adult with developmental and other disabilities; transfer a person to and from wheelchairs; to and from beds; on and off toilet seats; in and out of showers; and, in and out of vans or other vehicles. An employee in this position must be able to physically intervene (when appropriate and in an approved manner) to protect a person from causing harm to themselves or others and to safely evacuate the community residence/van/facility during an emergency. These duties may entail occasionally exerting 50-70 pounds of force by pulling, pushing, or lifting objects or people. The employee will collaborate with people who possess diverse disabilities and may exhibit a spectrum of behaviors, potentially including assertive and unpredictable behavior. Employees will regularly be required to stand; walk; sit; use hands (or hands and arms) to carry, finger, handle or feel objects, tools, or controls; reach with hands and arms; climb stairs; balance; stoop; crouch and kneel. Employees will also be regularly required to hear (with or without hearing devices), see (with or without corrective lenses), and speak.

  • Requirements: Access to reliable transportation. Active valid driver’s license, a clean driving record, and no more than two (2) points. Maintain proof of current automobile insurance. Maintenance of all required trainings under DDA guidelines and The Arc Northern Chesapeake Region policies and procedures. Flexibility of scheduling to meet agency and program needs. Ability to obtain DOT Medical Card (Non-CDL) within thirty days (30) of employment.

Requirements

A high school diploma is required, while an associate degree in human services and 1-3 years of experience with individuals with disabilities are preferred. Candidates must have a valid driver's license, reliable transportation, and the physical ability to assist individuals with mobility needs.
