- We are searching for a Community Health Worker to promote access to services, provide health education and assist community members in navigating the health care and social services systems.
- PRINCIPAL DUTIES AND RESPONSIBILITIES: Creates connections between vulnerable populations, the health care system and community/state resources.
- Conducts intake assessment with ChristianaCare respite bed patients and maintains required patient caseload.
- Maintains duties at location, New Castle County Hope Center as well as assists patients in the community and clinical setting.
- Visit patients at least once a week or as needed.
- Frequent travel to community, patient’s homes, main office and hospital.
- Connects to internal and external resources using the Unite Delaware platform and resources provided at the Hope Center.
- Document patient engagement accurately and timely.
- Help patients identify socioeconomic issues that affect their overall health and develop health/social management plans and goals.
- Facilitates healthcare and social service system navigation.
- Establishes and maintains professional relationships with community leaders, faith leaders and others responsible for providing services to the community.
- Participates in community events promoting the Community Health Department.
- Educates staff, health providers and stakeholders about community needs.
- Provides culturally appropriate health education on health-related topics.
- Advocates for underserved individuals to receive appropriate services.
- Works to reduce cultural and socio-economic barriers between clients and institutions.
- Maintains professional competence by participating in local and national training sessions and conferences.
- MINIMUM EDUCATION AND EXPERIENCE REQUIREMENTS: High school diploma required; bachelor’s degree preferred One-year community and patient experience required, more preferred.
- Bi-lingual language skills are a plus.
- Public speaking, group facilitation and excellent writing skills preferred.
- Demonstrated ability to work with diverse populations required.
- KNOWLEDGE, SKILL, AND ABILITY REQUIREMENTS: Knowledge and skills to provide education appropriate to target audiences.
- Knowledge of community resources in New Castle County.
- Knowledge base in health education.
- Effective verbal skills to reach diverse racial, socio-economic and literacy levels of target audiences.
- Flexibility to adapt to a variety of program schedules, physical settings and target audiences.
- Ability to work with minimal supervision.
- Computer skills (MS Office, MS Word, MS Outlook, Teams, Excel, Power Point, and e-mail desirable) Strong Interpersonal Skills and Team player.
- SPECIAL REQUIREMENTS: Access to personal vehicle.
- Ability to work flexible schedule, including some nights/weekends as needed.
- Knowledge of New Castle County.
- WORKING CONDITIONS: Hope Center facility and some office environment.
- Moderate to extensive off-site travel within New Castle County, during normal working hours, with occasional and weekend hours required.

- Hourly Pay Range: $25.52 - $38.28 This pay rate/range represents ChristianaCare’s good faith and reasonable estimate of compensation at the time of posting.
- The actual salary within this range offered to a successful candidate will depend on individual factors including without limitation skills, relevant experience, and qualifications as they relate to specific job requirements.
