Job Summary

  • Provides outpatient clinical care to patients suffering from acute or chronic hyper/hypo glucose management, managing therapeutic regimes according to protocols.
  • Provides individualized or group, goal directed diabetes management education for patients and their families according to the Scope of Practice, Standards of Practice, and Standards of Professional performance for RN Diabetes Educators.
  • Provides diabetes education to patients, caregivers, staff, and the community.
  • Coordinates the Diabetic Care Center utilizing knowledge of educational needs, class development and presentations in a health care environment.
  • Uses instructional responsibilities and teaching opportunities to improve the quality of care.
  • Develops selected core curriculum and educational program planning, implementation and evaluation.
  • Assists in further development of diabetes care center.
  • At facilities in the system with recognized DSMES Programs, if applicable this position is the quality coordinator for the DSMES and will oversee effective implementation, evaluation, tracking, and reporting of DSMES service outcomes.

Essential Functions

  • Provides diabetes self-management education and training to patients with diabetes and their family members. Functions as a professional Diabetes Program Instructor. Provides individualized or group, goal directed diabetes management education for patients and their families according to the Scope of Practice, Standards of Practice, and Standards of Professional performance for Diabetes Educators. Provides diabetes education to patients, caregivers, staff, and the community.
  • Works collaboratively with multi-disciplinary diabetes care team to implement plan of care. Participates in the development of training materials. Provides clinical care to patients suffering from acute or chronic hyper/hypo glucose management, managing therapeutic regimes according to protocols. Provides educational interventions that utilize primarily interactive, collaborative, skill-based training methods and maximizes the use of interactive training methods.
  • Reinforces diabetes education and instruction to staff nurses and support staff. Develops and distributes educational materials to community. Develops a Plan of Care that will include, but is not limited to, self-care skills, disease process, medication, diet, activity, home safety management and emergency procedures. Provides Medical Nutrition Therapy to outpatients with diagnoses other than diabetes.
  • Conducts a follow-up assessment upon completion of the Diabetes Self-management Training, using program outcome measures. Contributes to, and participates in, a continuous quality improvement process regarding the Diabetes Self-management Program to measure, to identify, and address opportunities for improvement. Provides documentation to physicians on patient progress.
  • Collects and analyses specific client data and outcomes. Assists in the development and implementation of educational and dietetic counseling interventions based on client-specific needs.
  • Performs other job-related duties as assigned.
