  • GENERAL
  • DESCRIPTION: Provides individuals a community-based program with support services designed to promote the self-determination, independence, productivity, integration and inclusion in the community. The position accomplishes this through culturally competent programs aimed at providing on-site assessment, training, and supervision to individuals in the community, in employment situations and in house training activities in a manner reflecting a professional, supportive manner with the individual businesses and the community at large and in keeping with the Journey Found mission.

  • RESPONSIBILITIES: Applies age and population specific competencies. Understands and applies competencies specific to the general characteristics of each age group and specific population/individual served. Implements program safety protocols. Implements program safety protocols as necessary. Documents critical incidents. Completes appropriate report form following specified incidents as per agency and state policy. Interacts effectively and professionally with all stakeholders. Interacts effectively and professionally with co-workers, managers, People Supported, parents and members of the community and follows the professional standards of the program and company. Interacts appropriately with People Supported. Interacts in a manner that encourages individual participation, is helpful, supportive and respectful of differences with regard to the age, gender and culture of the individual being served. Manages groups of individuals in the program and the community. Able to work with more than one individual at a time, encouraging participation in the program, providing support and offering assistance in a manner that is respectful. Transportation. Provides transportation of People Supported within the community as required. Operates company vehicles safely and defensively. Operates vehicles in a safe manner at all times. Reports vehicle maintenance needs to supervisor. May be required to use personal vehicle to transport individuals. Adheres to company driving policy and GPS policy. Manages behavioral crises. Applies crisis prevention and intervention strategies following standard, approved intervention techniques and protocols as needed. Assists with personal care. Assists individuals served with activities of daily living in a safe, sensitive and respectful manner. Activities of dialing living include but are not limited to feeding/eating, bathing, dressing, toileting, transferring (walking), continence, budgeting. Plan development and evaluation. Develop vocational and non-vocational day services in accordance with the individual’s plan and preferences. Assesses consumers’ skills and behaviors and assists in developing Individual Plans. Evaluates the effectiveness of the goals, objectives, plans and procedures implemented. Provides case management to individuals as assigned. Implements skills building programs. Implements formal skill building programs to help individuals learn new skills and gain independence. When applicable, provide training and support in all aspects of employment, including task analysis, developing accommodations, and accessing host company benefits. Implements behavior support plans. Implements formal behavior support plans to help individuals with clinically significant behavior support needs. Ensure that each individual receives the necessary support and advocacy to achieve success in his/her vocational and non-vocational day services plan. Implements day support goals. Facilitates the development of natural supports with supervisors and co-workers, ensures the completion of all contracted services, negotiates and advocates for reasonable accommodations or job modifications with employers, and provides on-going feedback to the individual served regarding performance. Maintains confidentiality. Maintains the confidentiality of individuals served. Completes training. Completes the training programs required to adequately perform the functions of the job and other training as required by their supervisor or Journey Found. Quality improvement. Participates in processes of continuous improvement to increase productivity, reduce inefficiencies and enhance the quality of services provided to the individuals served. Maintains program facilities. Maintains the program facilities by carrying out or assisting the individuals in the program to carry out housework and light maintenance. Communication. Provides written and oral information to staff and management in a timely manner. Follows written and oral instructions.
  • Attends all required meetings included but not limited to: vocational, IP, team and staff meetings. Flex schedule. The work schedule is based on the day services plan of the individual (s) served. There will be times when training or other special needs will occur earlier or later in the established workday. Attendance at these meetings and trainings is expected. Weekends, holidays and overnight late duties are not typically expected except as part of the established work schedule where individual plans call for these hours to be scheduled. Adheres to the mission, vision and values of Journey Found and conducts him/herself within standards of professional conduct of his/her program, division and the organization. Performs other duties, responsibilities and undertakes projects at any program or location as assigned by a supervisor, Director of Support Services or other member of management of Journey Found. Adheres to the Journey Found Policy & Procedure which protects the privacy and security of consumer health information. Maintains the security of electronic media and returns to his/her supervisor when appropriate.
