The Service Coordinator plays a key role in supporting children and families enrolled in the Birth to Three Program. This role manages an individualized caseload, ensures timely access to services, and strengthens family capacity by providing coaching, resource navigation, and consistent communication. As part of our Primary Coach Approach to Teaming (PCATT) model, the Service Coordinator partners closely with families, therapists, social workers, and educators to deliver family-centered early intervention services that promote each child’s physical, developmental, and social-emotional growth. Early Intervention & Family Support Partner with families and the PCATT team to develop and implement IFSP goals. Use coaching strategies to strengthen family capacity and connect them to needed resources. Maintain regular communication, including home visits and ongoing check-ins. Provides all family communication, coaching, and resource navigation in both English and Spanish. Service Coordination & Documentation Coordinate assessments, services, and transitions in alignment with Birth to Three requirements. Document all services accurately across required systems and complete compliant billing. Meet productivity expectations. Collaboration & Community Navigation Communicate effectively with internal team members and external providers. Support families in accessing community resources, medical providers, and early childhood services. Participate in transition planning and attend IEPs as needed. Compliance & Professional Standards Maintain required training, licensure, and adherence to Birth to Three, Medicaid/TCM, and agency standards. Uphold Penfield’s mission, values, and commitment to DEIB. Perform additional duties that support the program and organization. Qualifications Required Education and Experience Bachelor’s degree in social work, Early Childhood, Human Services, or a related field. Two years of experience providing case management, family support, or early intervention services to children and families. Experience facilitating or contributing to individualized family plans (e.g., IFSP, IEP, service plans). Experience working with multidisciplinary teams (therapists, educators, medical providers, social workers). Experience conducting or coordinating developmental assessments OR working within an assessment-driven service model. Experience maintaining documentation across multiple systems and following billing, compliance, or Medicaid-related requirements. Preferred Education and Experience Wisconsin Social Work License. Experience in a home-visiting program or early childhood intervention setting. Experience supporting families through resource navigation (medical providers, community agencies, social services). Experience supporting families through transitions. Fluency in English and Spanish (verbal and written). Required

Strong organizational skills, accuracy in documentation, and adherence to productivity expectations. Ability to maintain composure, empathy, and reflective practice in emotionally complex situations. Proficiency with electronic documentation systems, Microsoft Office, and learning new software quickly Ability to communicate fluently in English and Spanish, both verbally and in writing. Physical Requirements Ability to move comfortably within diverse home settings, including bending, carrying materials, and navigating stairs or tight spaces. Ability to lift or carry up to 50 lbs. when supporting families with child-related equipment. Ability to sit on the floor or at low heights during family interactions, and transition safely between positions. Reliable transportation, valid driver’s license, and vehicle insurance to complete community and home visits. Ability to travel in varied weather conditions throughout the year.
