Description Make a Meaningful Impact in the Lives of Families Join EBCAP as a Healthy Families America (HFA) Family Visitor and play a vital role in supporting parents and young children at critical stages of development. In this position, you will partner directly with families, helping them build strong foundations, access essential resources, and achieve their goals in a supportive, strength-based environment.

Job Description

The HFA Family Visitor provides home-based services to families, focusing on child development, parenting, health, and overall family well-being. This role is responsible for conducting structured home visits, completing assessments, and supporting families in achieving individualized goals aligned with Rhode Island Department of Health standards and the Healthy Families America model.

Key Responsibilities Family Engagement and Support Conduct regular home visits centered on parenting, child development, and family wellness Partner with families to establish goals and create actionable plans Provide strength-based, culturally responsive guidance and support Resource Coordination and Community Connection Connect families with internal programs and community-based services Develop and maintain referral networks to expand support options Guide families through available home visiting program options to ensure best fit Assessments and Documentation Complete required screenings and assessments in accordance with program guidelines Maintain accurate, timely documentation aligned with EBCAP and RIDOH expectations Collaboration and Program Participation Participate in reflective supervision, ongoing training, and quality improvement activities Collaborate with programs such as Head Start, WIC, Behavioral Health, and Health Centers to ensure coordinated care Support outreach and recruitment efforts to engage eligible families

Skills, Knowledge and Expertise Skills Strong interpersonal and communication skills (written and verbal) Ability to build trust and maintain professional boundaries Strong organizational and time management skills Ability to work independently while effectively using supervision Cultural competence and ability to work with diverse populations Basic computer and documentation skills Bilingual ability (Spanish, Portuguese, or Creole) is a plus Required High school diploma or equivalent Minimum of 3 years’ experience working with children and families Knowledge of early childhood development (birth to age five) Ability to maintain confidentiality and manage sensitive information appropriately Valid driver’s license and reliable transportation for home visits Preferred Bachelor’s degree in Human Services, Social Work, Child Development, Education, or a related field Infant Mental Health endorsement Experience working within evidence-based home visiting or family support programs
