The Health Coach provides coaching and support to Medicare and Medicaid eligible individuals by developing wellness plans and educating patients. They collaborate with care coordination teams to identify behavioral needs and overcome barriers to healthcare delivery.
Candidates must have a BA/BS in a health-related field and at least three years of experience in health or member education. Experience with dual-eligible populations and knowledge of Medicare/Medicaid programs are required.

This position is primarily remote. Candidates must have an appropriate home workspace and be available for occasional in-person team meetings. Must be located in Indiana
The LTSS Health Coach is responsible for providing coaching and support to individuals eligible for both Medicare and Medicaid. This role involves developing health and wellness plans, educating patients and their families and coordinating services to ensure the best possible health outcomes. The idea candidate will have a strong background in healthcare or social services, along with a passion for empowering individuals to take control of their health.
· Responsible for promoting health-related prevention and wellness education and programs. · Collaborate with Care and Service Coordination teams to fully address member needs from a person-centered standpoint. · Conducting behavioral assessments to identify individual members’ knowledge, skills, and behavioral needs. · Coordinate specific health coaching as directed by nurse case managers to address objectives and goals as identified during assessment. · Identify a coaching plan by using behavior- change principles to identify member barriers and develop ways to overcome those barriers. · Coordinate with nurse case managers to provide feedback on member goal attainment and clinical issues. · Monitor and evaluates interventions for effectiveness. · Will help raise awareness for the Pathway to Aging Program and assure members and providers stay informed of organization’s programs, policies and procedures and covered benefit services. · Will help identify and address barriers to an effective health care delivery system for members and providers.
· Must have a BA/BS in (such as health education, exercise physiology, respiratory therapy or dietician), minimum of three yeas of related experience in health education, exercise instruction, or member education. · Experience working with dual-eligible populations and deep understanding of Medicare and Medicaid programs. · Certified Health Coach (CHC), Certified Case Manager (CCM) or other relevant certifications are a plus.
M- F 8am to 5pm EST; Starting Salary $50, 000 to $70,000 (based on experience) Health Insurance and Benefits offered.
