For more than 50 years, NMHSI—a nonprofit community health center—has been committed to improving the health and wellness of communities across Northwest Michigan. We are seeking a compassionate and dedicated Part-Time Family Nurse Practitioner to provide high-quality primary care services at our Child & Adolescent Health Center (CAHC) located within TCAPS West Middle School in Traverse City, Michigan.

About the Role

The Family Nurse Practitioner provides comprehensive primary care services to students ages 3-21 in a school-based clinic, delivering patient-centered care through assessment, diagnosis, treatment, and follow-up. Working collaboratively with an interdisciplinary team, this role supports prevention, wellness, and continuity of care while helping students overcome barriers to accessing healthcare.

  • Key responsibilities
  • Provide comprehensive primary care services to students ages 3-21 in a school-based health center.
  • Perform physical assessments, diagnose illnesses, develop treatment plans, and prescribe appropriate therapies.
  • Conduct preventive health screenings and provide referrals for specialty or supportive services as needed.
  • Coordinate follow-up care and collaborate with families, school personnel, and healthcare providers to ensure continuity of care.
  • Document patient encounters accurately and maintain compliance with coding, HIPAA, FERPA, and organizational standards.
  • Participate in interdisciplinary care planning, case reviews, and collaborative practice meetings.
  • Support Child & Adolescent Health Center quality improvement initiatives and program requirements.
  • Promote health education, disease prevention, and wellness through school and community outreach activities.
  • Participate in on-call responsibilities and after-hours patient support as required.
  • Contribute to clinical policy development, staff education, and team-based care initiatives.

Qualifications

Required

  • Master's Degree in Nursing (MSN) from an accredited program.
  • Active Registered Nurse (RN) license in the State of Michigan.
