• Shift: DayEMPLOYER INFORMATION Clifford Beers is a mental health clinic serving children and families. Our mission is to provide integrated services addressing mental, physical, and social determinants in order to improve health, resiliency and quality of life for children, families and communities. PURPOSE Care Coordination is an integral component of the philosophy of care at Clifford Beers Community Care Center. The Child First Care Coordinator works collaboratively with the Child First Clinician as part of a dyadic team to deliver comprehensive, relationship-based, trauma-informed services to young children and their families in accordance with the Child First model. The Child First Care Coordinator partners with families to identify strengths, promote resilience, reduce barriers to care, and improve child and family well-being through family-centered, culturally responsive, home- and community-based services. The Care Coordinator collaborates closely with the Child First Clinician to address social determinants of health, strengthen the parent-child relationship, coordinate services across systems, and connect families with community resources that support healthy child development. Services include a combination of billable and non-billable activities based on each family's strengths, needs, priorities, and level of acuity. Services are delivered primarily in families' homes and community settings to maximize engagement, reduce barriers to care, and promote long-term success. As a member of the Child First interdisciplinary team, the Care Coordinator participates in reflective supervision, interdisciplinary case consultation, and ongoing professional development to ensure fidelity to the Child First model and delivery of high-quality, evidence-based services. This position requires a flexible work schedule, including three scheduled evenings per week until clinic closure at 8:00 p.m., to accommodate family availability and program needs.

• Differential: Employees assigned to the DCF Child First Care Coordinator position are eligible to receive a DCF Child First Program differential of $9,100 annually, paid in equal biweekly installments. Eligibility for this differential begins upon assignment to the position and is contingent upon continued funding for the DCF Child First program KEY RESPONSIBILITIES The essential functions of this position, include, but are not limited to: Interpersonal communication Establishing rapport with family members, schools and providers Engaging, educating, empowering families in a manner appropriate in fostering relationships that lead to the family’s success Tailor interactions to fit the tone, volume, and appropriate language for the client’s age and environment Collaboration and teamwork Collaborate and participate in internal and external meetings related to client care Regular participation in staff meetings, team meetings, case conferences, and external trainings as needed Developing a shared understanding of respective roles and responsibilities of team members to ensure efficient collaboration Facilitating and monitoring implementation of each Plan of Care developed by the family’s team Maintain a flexible schedule that includes three scheduled late evenings per week to provide services to families who are working and require appointments after 5:00 p.m. Care planning & coordination Engage families in the community and regularly conduct home visits Linking multiple services, healthcare providers, and community resources to meet families’ needs Ensuring flow and exchange of information among family members, the team, and linked providers/supports Documentation and Performance Maintain an active caseload of families to meet monthly, quarterly and yearly programmatic benchmarks and funder requirements Accurately document work in the organization’s electronic health record and other databases required by program Meet billable expectations as directed by the supervisor. Processing and determining appropriate eligibility Flex work hours to offer 3 late nights for families who require services after 5pm Ensure all communication and documentation are provided in accordance with organizational policies and in a manner that is culturally and linguistically appropriate for each family Perform related and appropriate duties as assigned Ensure documentation meets organizational standards, Child First program requirements, payer guidelines, billing requirements, and reimbursement expectations. Work collaboratively with the Child First Clinician as a dyadic treatment team to provide coordinated, relationship-based services to prenatal Caregivers and children birth through age five and their caregivers. Utilize Infant and Early Childhood Mental Health (IECMH) principles to support healthy social-emotional development and strengthen caregiver-child relationships. Partner with families using a strengths-based, family-driven approach that promotes family voice, choice, and self-determination. Address social determinants of health by assisting families in accessing healthcare, housing, food security, transportation, childcare, financial assistance, educational resources, and other community support. Coordinate services across multiple child- and family-serving systems to ensure integrated care. Build collaborative relationships with pediatricians, Birth to Three providers, schools, childcare providers, Early Head Start, the Department of Children and Families (DCF), behavioral health providers, primary care providers, and other community partners. Participate in reflective supervision, case consultation, Child First team meetings, and ongoing training to maintain fidelity to the Child First evidence-based model. Support parent-child relationship strengthening through education, coaching, advocacy, and connection to appropriate community resources. Promote family engagement and continuity of care by reducing barriers to treatment and encouraging active participation throughout the course of services. Maintain fidelity to Child First model standards and organizational quality improvement initiatives. TRAVEL This role may require travel throughout the State of Connecticut. QualificationsEDUCATION Bachelor's degree in Social Work, Human Services, Psychology, Sociology, Early Childhood Education, Public Health, or a related field preferred. An equivalent combination of education and relevant experience may be considered. EXPERIENCE Experience providing care coordination, case management, family support, advocacy, or community-based services to children and families is preferred.
