About the role

The Case Manager provides comprehensive, participant-centered support to youth ages 16-24, including intake assessments, individualized service plans, and career guidance. They also facilitate workshops, maintain accurate case files, and collaborate with internal and external partners to ensure participant success.

Description The YouthBuild Case Manager provides comprehensive, participant-centered case management and supportive services to YouthBuild participants ages 16-24. The Case Manager works closely with participants to identify barriers, develop individualized service plans, coordinate supportive services, and support educational, employment, and personal development goals throughout program participation.

Participant Case Management Conduct comprehensive intake assessments to evaluate participants educational, employment, housing, transportation, financial, mental health, and supportive service needs.

Develop Individual Service Plans with measurable goals and complete the job readiness portfolio.

Maintain ongoing case management to identify barriers to participation and coordinate appropriate interventions and referrals.

Support participants in achieving educational goals, including GED completion, high school diploma attainment, industry-recognized credentials, and post-secondary enrollment.

Monitor participant attendance, engagement, academic progress, credential attainment, and employment readiness.

Assist participants with career exploration, career planning, resume development, interview preparation, and employment readiness.

Encourage leadership development, civic engagement, and life skills growth consistent with the YouthBuild model.

Assist participants in transitioning to employment, apprenticeships, advanced training, military service, or higher education, in partnership with Employment and Career Pathways team members.

Participant Engagement Facilitate workshops related to job readiness, life skills, financial literacy, conflict resolution, goal setting, wellness, and leadership development.

Assist participants in developing problem-solving, decision-making, communication, and self- advocacy skills.

Participate in outreach and recruitment efforts as needed.

Documentation and Grant Compliance Maintain complete, accurate, and timely participant case files in accordance with organization/grant requirements.

Enter participant information into required information systems and databases.

Track participant outcomes and supportive service utilization.

Collaboration Collaborate with YouthBuild instructors, career coaches, construction staff, leadership, and external partners to coordinate participant success.

Participate in multidisciplinary team meetings, staffing discussions, and case reviews.

Requirements

Qualifications

Associates degree or equivalent experience in case management, education, social work, workforce development or other human service field. Minimum of two years of experience providing case management, youth services, workforce development, education, or related human services. Experience working with young adults facing barriers to employment and education and/or refugees, immigrants, and low English proficiency individuals is preferred. Valid drivers license and reliable transportation. Skills Needed Knowledge of case management principles and best practices. Understanding of youth development, trauma-informed care, motivational interviewing, and strengths-based practice. Knowledge of workforce development systems and career pathways preferred Excellent organizational and time management skills. Strong verbal and written communication skills. Ability to build rapport with diverse youth populations. Ability to maintain accurate records and documentation. Strong problem-solving and conflict resolution skills. Ability to prioritize multiple responsibilities in a fast-paced environment. Proficiency in Microsoft Office Suite and case management databases. Working Hours and Physical Effort Hours: Monday-Friday 8:00-5:00 pm, with possible evening events Some work performed in an interior temperature-controlled office, performing standard office tasks (sitting, typing) Regular work in the field at partner locations or meeting participants in community settings, use of personal vehicle with mileage reimbursement and travel-related exposure to changing weather Position requires occasional bending, squatting, sitting, standing, walking, pulling, pushing, lifting, and moving items that may weigh up to 10 lbs.
