  • Details: General Notes Dell Medical School is seeking a Behavioral Health Care Coordinator. This position has an end date of 8/31/2027, but may be renewable based upon availability of funding, work performance, and progress toward goals. Funding is expected to continue. Purpose This role supports mental health programs and initiatives developed through the Texas Child Mental Health (TCMH) Care Consortium, with a focus on improving the quality and accessibility of mental health care for children and adolescents across Texas. The Behavioral Health Care Coordinator plays a vital role in supporting children and families through TCMH programs like Texas Child Health Access Through Telemedicine (TCHATT), a statewide initiative that partners with school districts to provide telemedicine and telehealth services aimed at identifying and assessing the behavioral health needs of children and adolescents, and connecting them to appropriate mental health resources. Reporting to the Behavioral Health Support Supervisor, the Coordinator helps children and families navigate through TCMH programs such as TCHATT, assists in connecting them to providers in the community if needed, and maintains confidentiality of information in all aspects of care.

Responsibilities

Interdisciplinary Collaboration and Clinical Documentation ● Works in an interdisciplinary model, collaborating with pediatric psychiatrists, psychologists, licensed mental health professionals, and operational staff to support students and families receiving care. ● Assists in navigating treatment recommendations and assembles clinical background and referral information. ● Schedule, cancel, and reschedule patient appointments. ● Completes accurate and timely documentation in the clinical record in accordance with professional ethics and federal/state regulations. Community Resource Coordination and Stakeholder Support ● Identify and, as clinically indicated, connect students and families to appropriate community resources for long-term care. ● Provides ongoing support to ensure effective transitions and minimize barriers to care. ● Coordinates with professionals, schools, and families to enhance satisfaction and service delivery. ● Serves as a liaison for records management, and general inquiries related to program-specific services. Communication and Referral Management ● Oversees internal and external communication regarding clinical services, discharge planning, and school collaboration. ● Supports dissemination of referral information and documents interactions in the electronic medical record. ● Collaborates with designated program staff in utilization of identified community resource database and assists in problem-solving complex cases. Training and Special Projects ● Develops and delivers trainings for new hires . ● Participates in special projects as assigned. Other duties as identified by department Required Qualifications Bachelor’s degree in social work, education, psychology, or a related field. Three (3) years of professional experience working in a health care or educational (preferably K-12) setting. Excellent oral and written communication skills. Proficiency in electronic systems including, but not limited to, Google Suite, Trayt, Slack, Athena, and Calendly. Relevant education and experience may be substituted as appropriate.

Preferred Qualifications

Master’s degree in social work, psychology, education, or a related field. Knowledge of managed care, medical terminology, patient referral processes, claims, and CPT coding. Experience working with pediatric behavioral and mental health patient populations. Knowledge of educational practices supporting mental health and social-emotional learning in K-12 settings. Experience developing, refining, and evaluating patient care plans or student education plans. Able to prioritize, plan, and handle multiple tasks/demands simultaneously. Bilingual (English/Spanish) language proficiency. MARGINAL OR PERIODIC FUNCTIONS: ● Participates in interdisciplinary team meetings, case reviews, and planning sessions to support collaborative care. ● Assists with data collection and reporting for program evaluation, patient outcomes, and referral trends. ● Contributes to quality improvement initiatives by identifying service gaps and recommending enhancements to care coordination processes. ● Provides backup support for other care coordination staff during absences or periods of high service demand. ● Performs related duties as required KNOWLEDGE/SKILLS/ABILITIES Interpersonal Savvy ● Relates well to all kinds of people; builds appropriate rapport; listens; uses diplomacy and tact. ● Builds trust with families and providers. ● Navigates sensitive conversations with empathy. ● Resolves conflicts between stakeholders. Minimum Qualifications

Requires a Bachelor's Degree in Social Work, Education, Psychology, or a related field with at least 3 year(s) of experience in health care, governmental, educational (preferably K-12), or other related setting . Proficiency in electronic systems including, but not limited to, Google Suite, Trayt, Slack, Athena or Epic, Calendly.
