Health Coach

The health coach will act as the primary contact for patient health and wellness matters, responsible for providing tools and motivation to improve health outcomes within the onsite primary care team. Key duties include greeting patients, taking vitals, hosting educational sessions, reinforcing doctor's recommendations, and participating in outreach efforts.

Candidates must possess a high school diploma or ged, with a bachelor's in nutrition & dietetics preferred, alongside a minimum of two years of experience in nutritional services. Fluency in both written and verbal spanish is required for this role.
