Community Support Facilitator

The community support facilitator provides support and leadership for consumers to engage in community activities as defined by their individualized personal plan. Responsibilities include facilitating relationships, advocating for consumers, and ensuring safety through behavioral interventions.

Candidates must have a high school diploma or equivalent and two years of related experience working with people with disabilities. A valid california driver's license and a clear driving record are also required.
