Care Coordinator

The care coordinator coordinates child development monitoring and intervention services for children aged 0-3 years with developmental delays or risks, modeling strategies and providing education to parents and families to promote development. This role involves collaborating on the individualized family service plan, meeting funder guidelines, managing a caseload, and facilitating team meetings while ensuring timely documentation and service delivery.

A bachelor's degree in social work, social service, or education is required, with a minimum of one year of professional social work experience and experience with children with special needs in early intervention being highly preferred. Candidates must possess significant knowledge of developmental disabilities, familiarity with ada and idea guidelines, and strong communication skills to interact effectively within a primary service provider model.
