Health Coach and Educator (Field Remote, Home Visits in Raleigh, NC)

The health coach and educator will conduct home and virtual visits to establish wellness coaching relationships with patients, caregivers, and families by providing support, encouragement, and education to work towards better health. Key duties include contacting eligible patients for the care at home program, collaborating on action plans with clinicians, encouraging healthy habits, and assisting patients with accessing community resources.

Required qualifications include a minimum of one year of experience as a wellness or health coach/Educator and a high school diploma or ged (An associate's degree or two additional years of experience can substitute for the degree). Candidates must possess strong communication, interpersonal, and problem-Solving skills, along with knowledge of lifestyle topics, nutrition, exercise, and the ability to work independently while maintaining strict hipaa confidentiality.
