Case Manager

The case manager will engage youth and families quickly, serving as the primary contact for up to 90 days to conduct assessments and develop individualized service plans. Responsibilities include providing advocacy, coaching, connecting youth to community resources, tracking progress, and maintaining accurate documentation.

A bachelor's degree in social work or a related field is required, along with a minimum of one year of experience working with at-Risk youth in the community. Candidates must have access to a vehicle, a valid driver's license, insurance, and be available to work saturdays as needed.
