Healthy steps community health worker

The community health worker will serve as the healthysteps specialist, providing one-On-One support, education, and needs assessment to enrolled families to promote infant and child development and well-Being. This role requires close collaboration with the pediatric team to connect families with necessary healthcare, social services, and resources while tracking progress and advocating for their needs.

Candidates must possess a high school diploma or ged, with an associate's degree or chw certification preferred, and must meet the requirements or hold certification as a community health worker. Essential qualifications include bilingual proficiency in spanish/English or haitian creole/English, strong communication skills, and experience working with diverse, low-Income populations, preferably in maternal and child health.
