Core responsibilities
The family case manager collaborates with families to facilitate a team process, ensuring families have voice and choice, aiming for youth with behavioral health issues to live in stable family or community settings. Facilitators address concerns, create safety plans, and build support systems to promote resiliency, maintain or reunify families, and prevent out-Of-Home placements.
Requirements summary
Candidates should possess a bachelor's degree in social work or a related field, or three years of relevant experience, which can include applicable lived experience. Required experience includes working with youth with behavioral health issues, county systems, and culturally diverse populations in lower socio-Economic conditions.
