Outpatient Therapist

Provide in-Person outpatient therapy and complete comprehensive evaluations in alignment with ccbhc requirements. Collaborate with multidisciplinary team members to develop individualized treatment plans and support timely access to care.

Experience in a ccbhc or integrated care setting is preferred, along with familiarity with rapid intake models. Knowledge of minnesota behavioral health statutes and documentation standards is also required.
