YMCA of Greater Indianapolis - Chronic Disease Prevention - Community Health Worker

The community health worker supports individuals and families through health education, outreach, and connecting them to resources to enhance overall health outcomes. This role focuses on delivering the diabetes prevention program and enrolling participants in chronic disease prevention programs to support long-Term physical health.

A high school diploma or equivalent is required, and candidates must possess or agree to obtain community health worker certification within six months. Preferred qualifications include one year of experience leading classes, working with special populations, and knowledge of health and wellness principles.
