Care Coordinator - Children - Pauls Valley Oklahoma

The care coordinator provides individualized case management, rehabilitative, and wellness services to promote psychological change, alleviate mental disorders, and facilitate recovery, often involving scheduling and facilitating wraparound family team meetings. This role requires completing comprehensive assessments, care plans, crisis plans, and ensuring timely data entry while monitoring consumer needs and assisting them in utilizing community resources.

Candidates must meet specific educational requirements, which include a bachelor's or master's degree in a behavioral health field, or equivalent experience/Licensure, and must obtain behavioral health case management and wellness coach certifications within three months of employment.
