Care Coordinator - Children

The care coordinator provides individualized case management, rehabilitative, and wellness services to children and families to facilitate recovery and prevent out-Of-Home placement. They are responsible for coordinating wraparound team meetings, completing clinical assessments, and developing care and safety plans.

Candidates must hold a degree in a behavioral health field or possess relevant experience working with individuals with mental illness or substance use issues. The role requires obtaining specific behavioral health certifications within the first three months of employment.
