The physician will provide comprehensive care in a 100 percent outpatient family medicine setting, managing preventative services, chronic disease management, and acute care within a collaborative team structure. Candidates must be board certified or board eligible in family medicine and eligible for illinois medical licensure, possessing an active dea license or the ability to obtain one. Strong clinical skills in outpatient primary care and a commitment to patient-Centered, team-Based care are essential.
