Life Coach - Independent Living Case Manager

The life coach/Case manager will work with young adults to successfully transition them to adulthood by equipping them with necessary services, supports, and resources for independence. Key duties include facilitating treatment plans, managing client placements, ensuring legal documentation, and providing ongoing training and support across various community settings.

Candidates must be at least 21 years old, possess a valid driver's license with required insurance, and have either an lsw/Lpc or a bachelor's degree in a human services field plus two years of experience working with young adults with emotional or behavioral problems. The role also requires proficiency in various software, the ability to lift up to 50 lbs for apartment setup, and successful completion of background checks and screenings.
