Wraparound facilitator (76091)

The facilitator works with youth and families across multiple settings to develop, implement, and monitor a wraparound plan by convening necessary team members. This role involves continuous collaboration with agencies and organizations to secure required supports and services for the youth and family.

Candidates must possess a bachelor's degree in a human services field along with two years of relevant experience, and hold a valid driver's license.
