Wraparound facilitator (74922)

The wraparound facilitator coordinates and facilitates team meetings to develop and monitor individualized care plans for youth and their families. They provide direct support across various settings, including homes, schools, and the community, while advocating for the family's needs and managing crisis situations.

Candidates must possess a bachelor's degree in a human services field and at least two years of relevant experience. A valid driver's license is required, and experience working with children and adolescents with mental health or substance abuse issues is preferred.
