Wraparound Facilitator 2K Sign On Bonus

The wraparound facilitator implements the wraparound process for youth and families, coordinating and individualizing services to safely reduce reliance on congregate care while supporting family strengths and community involvement. Responsibilities include consulting with the program manager, ensuring contract compliance, facilitating team meetings, developing care plans, tracking progress, and responding to client crises.

Candidates must possess a bachelor’s degree in social work, counseling, psychology, or a related field, along with two years of post-College work experience providing direct services to children and families. Required knowledge includes mental illness diagnoses and behavioral disorders in children, along with the ability to collaborate effectively and maintain a flexible schedule.
