PAT Home Visitor

The home visitor is responsible for planning, implementing, and reporting on home visits, family engagement activities, connecting families with resources, and conducting recruitment and community outreach. Duties include completing required home visit frequencies, conducting developmental screenings and family assessments, and maintaining accurate documentation and data entry.

Required qualifications include a high school diploma or ged plus two years of experience working with young children and/Or families. Preferred qualifications include a child development associate (Cda), three years of relevant experience, or an associate's or bachelor's degree in social work, early childhood education (Ece), or a related field.
