NFP Nurse Home Visitor - Lake County - FT

This role involves developing therapeutic relationships with first-Time parents through home visits, applying the nurse-Family partnership (Nfp) model to coach and support them from pregnancy until the child is age two. Responsibilities include managing a caseload of up to 30 clients, ensuring adherence to clinical standards, and accurately documenting progress in the nfp information management system.

Candidates must possess a bachelor’s degree in nursing (Bsn) and hold an active indiana registered nurse (Rn) license, with public/Community health or home visiting experience being highly preferred. Essential requirements include reliable transportation, the ability to travel daily for home visits, and passing a criminal background check and drug screen.
