Core responsibilities
The lifestyle coach is responsible for facilitating the national diabetes prevention program (Ndpp) through virtual or in-Person delivery, which includes recruiting participants, leading the yearlong lifestyle change program, and reinforcing key curriculum concepts. Essential functions also involve collecting and recording participant data weekly for submission to the dprp and maintaining accessibility and support for participants.
Requirements summary
Minimum qualifications require a high school diploma and two years of experience in a health-Related field, or an equivalent combination of education and experience. Candidates must achieve hipaa certification and diabetes prevention program lifestyle coach certification within 90 days of hire, possess strong communication and organizational skills, and be willing to consent to a criminal background check and drug screening.
