Conducts educational assessment on school-aged patients to determine their educational needs and to develop an educational plan. Evaluates how each patient learns best and adapts teaching methods to fit their unique learning needs. Develops and plans creative teaching and learning activities. Incorporates and facilitates educational community programming and activities for patients. Collaborates as an integral part of an interdisciplinary team to ensure the patient's educational needs are anticipated and addressed in a thorough, proactive and effective manner. Implements discharge planning process in conjunction with the treatment team. Coordinates transition plan for return to school, providing services to ensure a smooth transition for the child back to home and school. Travel may be required. Qualifications Bachelor's degree in education or related field required. One year of classroom teaching experience or hospital school teaching preferred.
