Care Coordinator - Youth

The care coordinator will collaborate closely with children, their families, and community partners to connect youth facing behavioral or developmental health challenges to necessary care and services. This role involves working directly with children/Youth and their families in community-Based settings.

Candidates must meet educational requirements which range from a high school diploma with 3 years of experience up to a master's degree with 1 year of experience. Required expertise includes knowledge of family systems, community resources, case management, or child development, along with possessing a valid driver's license and reliable transportation.
