Grant Home Visitor - St. Vincent Medical Center

The primary function is providing culturally sensitive home visitation services to expectant mothers and families with young children to strengthen parent-Child relationships, promote healthy development, and enhance family functioning. This includes delivering educational materials, providing referrals, linking families to medical providers, administering developmental screenings, and maintaining accurate case records.

A bachelor's degree in social work, early childhood development, teaching, nursing, or a related human services field is required, along with preferably two years of recent community experience emphasizing home visiting. Candidates must possess reliable transportation, a valid driver's license, proof of insurance, and must obtain and maintain a home visitor credential during orientation.
