NFP Nurse Home Visitor - St Joseph County

The nurse home visitor is responsible for developing therapeutic relationships with clients and their families, conducting home visits, and implementing the nurse-Family partnership model. They will assess clients' needs, provide education and support, and monitor client progress toward goals.

A bachelor’s degree in nursing and a registered nurse license in indiana are required. Experience in public health, maternal/Child health, and home visiting is preferred, along with strong communication skills and the ability to work with diverse populations.
