NFP Nurse Home Visitor - Lake County - FT

This role involves developing therapeutic relationships with first-Time parents through home visits, applying the nurse-Family partnership (Nfp) model to coach and support them from pregnancy until the child is age two. Responsibilities include maintaining high standards of clinical nursing practice, managing a caseload up to 30 clients, and ensuring adherence to nfp guidelines and policies.

Candidates must possess a bachelor’s degree in nursing (Bsn) and hold an active indiana registered nurse (Rn) license, along with reliable transportation for daily travel. Preferred qualifications include experience in public/Community health, maternal/Child health, or home visiting, and bilingual skills in english and spanish are a plus.
