Core responsibilities
The care coordinator and group facilitator will engage and assist youth and their support systems in forming a child and family team to address identified goals. They will provide community-Based trauma-Informed services and connect clients to appropriate resources.
Requirements summary
Candidates should have direct lived experience with foster care or related services and preferably a bachelor's degree in a relevant field. At least 2 years of experience working with system-Involved youth is required.
