SERVICE COORD/HOME VISITOR-BSW/Full Time-Days/Miami Valley Hospital

The service coordinator/Home visitor assists families in obtaining necessary services and integrates direct services with those provided by other sources. They conduct assessments, collaborate with families and agencies, and facilitate transitions to preschool services.

Candidates must have a bachelor's degree in social work and at least 1-3 years of experience working with young children and their families. A licensed social worker (Lsw) is preferred, along with effective communication skills and a valid driver's license.
