Transitional Services Specialist

The specialist provides direct case management and transitional services to youth, including employment, housing, and educational support. They also build relationships with community providers and maintain accurate participant data to ensure program goals are met.

Candidates must have a bachelor's degree in a related field or an associate's degree with 2-3 years of relevant experience. A valid driver's license and experience working with high-Risk adolescents are required.
